This is the commonest form of hyperhidrosis. Severe excessive sweating affecting the hands tends to affect people in the 20—30 years old group. It is more common in females. It may also present in teenagers.
In severe cases the sweat literally drips off the hands and is extremely socially debilitating. Social contact may become highly embarrassing and patients often report difficulties with personal relationships. Everyday tasks such as writing also become very difficult.
Unfortunately, there is no simple conservative treatment for palmar hyperhidrosis. Botulinum toxin injections into the palms has been tried but is potentially very painful and has to be done under general anaesthesia. Also, temporary paralysis of the small muscles of the hands may occur as well as the fact that the treatment will need to be repeated after 4-6 months in at least two-thirds of patients.
Iontophoresis is another option which may work reasonably well in some patients with mild-moderate sweating.
The best treatment for severe palmar hyperhidrosis is Endoscopic Thoracic Sympathectomy (ETS) which is successful in at least 95% cases. |
